Overview
Associate Community Outreach
Schedule: Monday–Friday, 8:00am–4:30pm
Work Model: Telephonic case management with required home visits - Must reside in Indiana
Mileage reimbursement available
Role Overview
Supports patients with dementia and their caregivers. Experience with ageing population care and caregivers is preferred.
Key Expectations Within 12 Months
Effectively screen, assess, and onboard patients into The program.
Maintain and manage an active patient caseload.
Serve as a resource and support partner to team members to ensure high‑quality service for the program population.
Top Responsibilities
Serve as a bridge between the community and healthcare, government, and social service systems.
Assist individuals, families, and groups in accessing resources; provide informal counseling, support, and follow‑up.
Coordinate frequently with CMS GUIDE Model team members and IU Health providers on behalf of patients.
Support community outreach strategies, initiatives, and events that meet identified community needs.

Onboarding Experience
Cerner registration training.
Video modules covering CMS GUIDE Care model and dementia education.
Independent review of onboarding materials.
Shadowing of team members to learn operational tasks.
Progression from shadowing → leading with support → leading independently.
Required Qualifications
Bachelor’s degree or equivalent years of relevant experience.
0–3 years of experience
Ability to work in a dynamic and complex business environment.
Demonstrated ability to manage multiple tasks, events, or projects simultaneously.
