Qualifications:
- Masters Degree in Social Work or Counseling
- Licensure in your field of study (LLMSW, LMSW, LLPC, LPC)
- At least one year providing behavioral health services to children
- Valid Michigan Drivers License
Duties and Responsibilities:
- Provides intensive Home Based individual, group, conjoint or family psychotherapy, case management and support services to children with severe emotional disturbance and their families.
- Conducts clinical assessments, develops diagnoses, and participates in developing person/family centered treatment plans with client families. Monitors client progress and coordinates appropriate mental health and other needed services.
- Responsible for maintaining an electronic and paper chart which includes required documentation of services provided, progress notes, treatment plans, periodic reviews and client rights documents.
- Participates in team meetings and one hour of weekly individual supervision to review progress, identify barriers and develop strategies for better assisting families in reaching their goals.
- Participate in utilization review committee meetings for the purpose of reviewing case records for contract compliance, quality assurance, appropriateness of services and treatment progress.
- Provides 24/7 crisis intervention availability to help offset potential risk of self-harm or conditions which could lead to out of home placement.
- Ensures that paperwork is maintained in accordance with Medicaid and Department of Community Health guidelines, as well as Easterseals policies and procedures.
- Responsible for assessing, reviewing all requests for services and authorizing medically necessary services for all consumers.
- Demonstrates knowledge of principles of trauma informed care.
- Willingness to support and assist consumers with trauma related issues.
- Ensures that all individuals served caseload have CAFAS/PECFAS completed quarterly.
