Remote RN Case Manager (Costco Team)

Location: Remote (Preferred: San Antonio, TX area) Travel to the San Antonio office may be required if you live within 45 minutes of their location.
Schedule: Monday–Friday, 8:00 AM–5:00 PM CST (occasional 10:00 AM–7:00 PM rotation)
No weekends or holidays

About the Role

Join our dedicated Costco Team as a Remote Case Manager RN, where you’ll play a key role in improving members’ health outcomes. In this role, you’ll assess, plan, and coordinate care—helping members navigate their healthcare needs and achieve better overall wellness.

What You’ll Do

• Conduct comprehensive telephonic (and occasional in-person) assessments
• Develop and implement personalized care plans
• Identify and address clinical, social, and behavioral health needs
• Collaborate with multidisciplinary teams to support holistic care
• Guide members through benefits, resources, and care transitions
• Use clinical tools and data to reduce risks and improve outcomes

What You’ll Bring - REQUIRED

• Active, unrestricted multi-state RN license in state of residence
• 3+ years of clinical RN experience
• 1+ year experience using multiple systems (e.g., Excel, Outlook, web-based tools)
• Strong assessment, communication, and critical thinking skills
• Willingness to obtain additional state licenses (company-paid)

Preferred Qualifications

• Case management experience (integrated model a plus)
• Bilingual (Spanish/English)
• BSN preferred

Pay Range

The typical pay range for this role is:

$60,522.00 - $129,615.00

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.  The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.  This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
