This position is in the Colorado Department of Public Health and Environment, Disease Control and Public Health Response Division, has been recommended for reallocation to Public Health and Community Outreach IV.
  
This announcement represents the selection process for an occupied, reclassified state position. This position is open only to classified permanent employees of Disease Control and Public Health Response Division within the Colorado Department of Public Health and Environment. Applications will not be considered from any individuals not specifically covered by these strict eligibility requirements.  

IMPORTANT INFORMATION:
  • This position will have hybrid workplace options available but will still be required to report to the listed address at the discretion of the supervisor and based on business needs.
  • Critical Status: This is a critical position and can be expected to work and/or remain at their worksite in delayed start,
    early release, or closure situations.

Description of Job

The Work Unit:

The Health Equity Branch partners with community-based organizations, healthcare entities and local public health agencies (LPHAs) in Colorado by promoting health equity and implementing strategies tailored to address the varying complex causes of health disparities that often stem from structural racism or the historical disenfranchisement and discrimination of marginalized groups, including racial and ethnic minorities, low-income populations, people living with disabilities, people experiencing homelessness and members of the LGBTQ+ community among others.

The Health Equity Branch works collaboratively to set strategic priorities, collect and disseminate data, identify and implement effective strategies, and align resources within the division and across the department and other state agencies. The Health Equity Branch includes the Community Action and Engagement (CA&E) Unit. The team’s goal is to ensure that all Coloradans have access to adequate health care services and resources to improve health and well-being. They do this by creating regional- and population-specific support networks that aim to advance health equity and address social determinants of health. This Unit consists of several lines of work focused around establishing a regional support network for local communities, primarily addressing the impacts of communicable diseases, as they relate to community health and supporting access to immunizations. CA&E activities are to include, but are not limited to, the provision of educational opportunities, referral for cultural/community health navigation, connection to wrap-around resource support, and in-person community engagement and outreach. The Community Action and Engagement team is responsible for being a resource to direct health equity programming across the state to assure community needs are met in populations experiencing a disproportionate impact of communicable diseases.

What You Will Do: Community Outreach Project Work Lead

This position serves to increase immunization rates by maintaining a Vaccine Champions program, and overseeing collaboration of this program with local public health agencies, providers, community organizations, and the public. This position interacts with clients, health care providers, schools, child cares and external stakeholders in a variety of ways. This position leads, supports and advocates for activities that are part of an ongoing effort to increase vaccine uptake among populations across Colorado. This position plays a key role in activities outlined in Colorado's Immunization Cooperative Agreement with the Centers for Disease Control and Prevention (CDC) aimed at increasing outreach to partners and external stakeholders. The position coordinates and works in parallel with the Community Action and Engagement Regional Coordinators and other staff to ensure cohesive coordination across the state; with a focus on services for the disproportionately affected populations. Their role is to understand health disparities and resources available to support disproportionately affected populations while assisting LPHAs, community-based organizations and other entities across the state in assessing and identifying barriers to communicable disease prevention. This position is responsible for ensuring coordinated support and resources within each region, when possible, including vaccine access and other disease prevention opportunities. Also, for identifying outreach opportunities where attendees may have an opportunity to engage with CDPHE to learn about public health issues affecting their communities, discuss community health needs, seek support, receive resources when available, and in some cases, receive immunizations. This position also leads branch efforts in building partnerships with local (primarily rural) providers to offer expanded services and preventative care at mobile public health clinic events across the state.

A combination of related education and/or relevant experience in an occupation related to the work assigned equal to Seven (7) years.
