About this role

The Full-Time Clinical Pharmacist position is essential to supporting both the operational and clinical service needs of Rx Partners At Home division as well as Hospice and Skilled Nursing Facility (SNF) pharmacy operations. In addition to prescription processing responsibilities, this position will provide critical clinical oversight and regulatory support to partner facilities through consultant pharmacist activities. The position will participate in weekend, evening, overnight, and on-call rotations, while supporting quality improvement initiatives and medication management programs within the long-term care setting. 


This position will work a combination of daylight and evening hours with a weekend and holiday rotation. 

Key Responsibilities

  • Lead assigned patient care services, clinical programs, and therapeutic initiatives to ensure safe, effective, and evidence-based medication use.
  • Collaborate with multidisciplinary healthcare teams to deliver patient-centered pharmaceutical care and optimize clinical outcomes.
  • Develop, implement, and evaluate pharmacy programs, stewardship initiatives, and medication-use strategies that enhance quality, safety, and operational performance.
  • Monitor and improve medication utilization practices through data analysis, quality improvement projects, and adherence to regulatory and accreditation standards.
  • Provide direct clinical pharmacy services, including medication therapy management, patient monitoring, medication reconciliation, discharge counseling, and emergency response support.
  • Identify medication-related problems and implement interventions to improve patient safety, therapeutic outcomes, and cost-effective care.
  • Serve as a clinical resource for physicians, nurses, and other healthcare professionals by providing evidence-based medication recommendations and education.
  • Support pharmacy operations, including medication procurement, dispensing, regulatory compliance, and oversight of pharmacy technician activities.
  • Develop and maintain policies, procedures, and best practices related to medication management and pharmacy services.
  • Contribute to the education and development of pharmacy students, residents, and staff through mentorship, training, and preceptorship activities.
  • Lead initiatives that advance pharmacy practice, improve medication-use systems, and support organizational goals for patient care excellence.

  • Graduate of an ACPE-accredited College of Pharmacy.
  • Bachelor of Science in Pharmacy required; Doctor of Pharmacy (PharmD) preferred.
  • Current pharmacist licensure in the applicable state required.
  • Completion of an ASHP-accredited PGY-1 Pharmacy Residency or a minimum of three years of clinical pharmacy experience.
  • Advanced knowledge of clinical pharmacy practice, comprehensive medication management, and evidence-based medicine within a specialized practice area.
  • Demonstrated ability to provide high-quality, patient-centered pharmaceutical care in a complex healthcare environment.
  • Board of Pharmacy Specialties (BPS) certification preferred.
  • Strong clinical judgment, communication skills, and ability to collaborate effectively with interdisciplinary healthcare teams.
