District Director of Clinical Services - SNF

Provides strategic clinical oversight and support of the day-to-day skilled nursing clinical community operations to Executive Directors, Healthcare Administrators, Director of Clinical Services, and Skilled Interdisciplinary Team, as well as district and regional team leaders above assigned communities. Supports the company goals and strategies and optimizes resources to ensure quality care and service delivery, training and development of staff, and management of risk and regulatory compliance.

1. Oversees regulatory compliance including reviewing survey preparedness/ management and outcomes, assisting with Plans of Correction and providing district/regional team support in the development and implementation of action plans

2. Completes internal and external investigations (i.e. Integrity line calls, complaint surveys, serious family concerns).

3. Oversees and trains Director, Clinical Services Specialists, HWDs/DCS as needed and other disciplines as assigned.

4. Responsible for the education, implementation, and maintenance of corporate policies and procedures, in accordance with state and federal regulatory guidelines.

5. Conducts regular quality audits through regional site visits and pre-survey audits of all health care areas to ensure compliance with Brookdale standards and state regulations, appropriate resident assessments, charges, and placement, appropriate staffing, staff training, and quality insurance.

6. Provides clinical guidance to Skilled associates, when necessary, to determine appropriateness for resident move-ins.

7. Promotes problem solving and utilizes community resources and support systems to address skilled resident needs.

8. Analyzes various Skilled healthcare reports to track and trend healthcare outcomes to assist the Regional Director of Clinical Operations in process improvement efforts.

Licensed as a registered nurse. Must have a valid driver’s license and access to a private vehicle for business travel. CPR Certification required.
