This position is located in Community Long Term Care (CLTC), Orangeburg County. 
This is an in-office role and not a telecommute or remote position.

Are you the One?
We are looking for an Eligibility Specialist I (CLTC) who works under general supervision of the Area Administrator. Provides technical support and assists the nurse consultant staff by performing pre-admission screening functions for applicants and participants seeking assistance from various CLTC (Community Long Term Care) programs. Assists with administrative support for the members of the nurse consultant and social work staff, vendors, provider case managers, and others as needed including assisting other CLTC offices with essential job functions according to program needs. Maintains a working knowledge of all CLTC programs and related systems including Phoenix information system.
  • Conducts referral processing activities.
  • Performs paraprofessional duties.
  • Promotes collaboration and effective communication.
  • Provides cross coverage assistance.
  • Processes Memorandum of Agreement (MOA) Pre-admission Screening Resident Review (PASRR).
  • Performs emergency preparedness duties or activities as required.
  • Performs other duties as assigned.

Position Duties

A high school diploma with clerical or paraprofessional experience.

Additional Requirements:

  • Sitting or standing for long periods of time.
  • Lifting requirements: 20 lbs.
  • Occasional travel, in and out of town, required for meetings and training.
  • In-office role.

Minimum Qualifications

  • Ability to communicate effectively both verbally and in writing.  
  • Excellent data entry and organizational skills.  
  • Knowledge of data preparation for computer applications.  
  • Ability to exercise judgment and discretion in interpreting and applying office practices and procedures.  
  • Knowledge of HIPAA regulations. 
  • Knowledge of and ability to use personal computer. 
  • Must be able to perform web-based software functions (keying data, run reports, update documents, etc.) 
  • The ability to multi-task within time frames and prioritize tasks. 
  • Phoenix application and workflow experience.
