SUMMARY (BASIC PURPOSE OF THE JOB)
Education: Graduate of an ACPE-accredited school of pharmacy. Doctor of Pharmacy (Pharm D) or equivalent advanced degree. Board of Pharmacy Specialties (BPS) certification required or must attain within two years of start date.
Experience: PGY1 Pharmacy Residency required; PGY2 Specialty Residency or three years equivalent specialty experience preferred. ED / Critical Care experience preferred.
Other Credentials: AHA ACLS-Adv Cardiac Life Supp; AHA BLS - Healthcare Provider; PALS Certification. Licensed Pharmacist in New Jersey. 2 hours of Stroke related Continuing Education contact hours annually
Knowledge and Skills: Possesses excellent organizational, interpersonal, conflict resolution, verbal, and written communication skills. Ability to effectively manage multiple projects and responsibilities simultaneously. Ability to respond quickly in a fast paced environment.
Special Training: ASHP-accredited pharmacy residency training required. Intermediate to advanced computer skills. ASHP accredited PGY2 Pharmacy Residency in Emergency Medicine or Critical Care preferred.

ESSENTIAL FUNCTIONS
- Perform real time medication order verification for all ED medication orders. Follow policies and procedures to maintain accurate patient medication profile. Initiate appropriate medication therapy based on age, disease, and drug considerations for neonatal, pediatric, adolescent, adult, and geriatric patients in the ED. Actively intervene to assure therapy is individualized based on medication factors and the patient’s physical and psychological needs.
- Identify and resolve problems with medication orders or therapeutic regimens. Clarify issues with prescribers and suggest order adjustments or alternative medication therapies when needed to prevent adverse events. Adhere to institutional policies.
- Attend all critical patient emergencies including Code Blue, Code White, RSI, Stroke Alert, Code PCI, Trauma Alert.
- Compound STAT medications at patient bedside to facilitate prompt medication administration.
- Provide comprehensive evidence based drug information to providers and ancillary staff in the ED and ensure proper drug utilization by adherence to formulary criteria and pharmacy policies.
- Perform medication history evaluation/reconciliation for identified patients in the ED admitted to the hospital based on qualifying criteria.
- Work collaboratively with other health care providers to implement clinical guidelines and policies to advance safe and effective medication use. Review protocols, orders, and policies as requested.
- Educate patients regarding safe and effective use of medications, possible medication side effects, and role of prescribed medications in overall therapeutic care plan as requested.
- Serve as a preceptor for Pharm.D. candidate student experiential education programs and PGY1 / PGY2 rotations, including didactic and experiential training in related practice areas.
- Participate in the professional development and competency of pharmacy staff and function as an educational resource for pharmacists and pharmacy technicians. Provide effective evidence-based medication and practice-related education to healthcare professionals and members of the department.
- Documents pharmaceutical care interventions in departmental reporting system. Actively identifies, mitigates, prevents, and reports potential and actual medication errors or adverse drug events. Recommends rational changes to medication systems as needed to prevent future problems. Implements, evaluates, and redesigns integrated evidence-based pharmaceutical care plans based on patient progress and evolving goals
- Identifies opportunities for improvement of medication-use system to advance patient safety, maximize therapeutic outcomes, or control costs. Participates in planning, implementing and evaluating new and existing programs. Maintains and enhances up-to-date evidence-based knowledge of drug and drug therapy by reviewing relevant professional journals and suggested readings.
- Demonstrates teamwork within Pharmacy Department and participate in group approach to patient care by providing support to peers and other practice areas as needed in addition to daily assignments.
- Participates in orientation, education, and mentoring of new staff (i.e. pharmacists, pharmacy residents, nurses, pharmacy technicians, etc.) as needed.
- Present current guideline directed pharmacotherapy information at interdisciplinary conferences
- Maintains a sustained record of contribution to total body of knowledge in pharmacy practice through presentations, publications, and professional affiliation activities. Maintains a professional profile and active membership in relevant pharmacy/medical organizations. Becomes a member in other local, state, or national professional organizations.
- Represent the Pharmacy Department at hospital meetings and actively participate in committees/subcommittees as related to the Emergency Department.
