Emergency Department Patient Navigator

Responsibilities

The ED Patient Navigator will be responsible for assisting in the coordination of the next level of care for patients that are treated in the SFIL Emergency Room, but do not have a designated PCP for necessary post discharge follow-up. Duties include assisting with access issues, identifying resources, providing educational material, and developing relationships with the community. The ED Patient Navigator coordinates the care in a timely and efficient manner, identifying and eliminating barriers to care.

Qualifications

EDUCATION, EXPERIENCE, TRAINING

 

1. Bachelor's degree or four (4) years of applicable cumulative job specific experience required or a licensed RN.
2. Current BLS certificate upon hire and maintain current.

A reasonable compensation estimate for this role, which includes estimated wages, benefits, and other forms of compensation, is $18.66 to $27.02. The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire, in which a wide range of factors will be considered, including but not limited to, skillset, years of applicable experience, education, credentials and licensure.
