Practices good clinical judgment. Progresses patient appropriately and safely during exercise. Provides education to patient and family regarding patient%27s disease process. Demonstrates awareness of patient%27s and family%27s readiness and ability to learn. %3C/li%3E%0A%3Cli%3EPerforms and documents physical, psychosocial, and emotional assessments during initial evaluation. Identifies appropriate interventions based on initial evaluation. Writes complete plan of care with specific, time based goals for each intervention. %3C/li%3E%0A%3Cli%3ERe-evaluates plan of care and exercise program to determine effectiveness. Utilizes critical thinking skills to monitor patient%27s progress. Reports any physical changes to physician in prompt manner. %3C/li%3E%0A%3Cli%3EPrepares for patient discharge. Assures that patient%27s and family%27s goals for rehabilitation have been met. Displays good knowledge of patient%27s resources and needs. %3C/li%3E%0A%3Cli%3EWorks to improve departmental standards of care through updating educational material, providing inservices to staff. %3C/li%3E%0A%3Cli%3EOrganizes patient treatment time and provides patient education. %3C/li%3E%0A%3Cli%3EDocuments all aspects of patient%27s care and continued need for rehabilitation. %3C/li%3E%0A%3Cli%3EReports errors promptly and takes appropriate follow-up action to rectify error. %3C/li%3E%0A%3Cli%3EServes as patient and family advocate. Responds to patient%27s and/ or family%27s complaints. Reports complaints to Manager or their designee in timely manner. Utilizes chain of command to resolve patient, operational, or personal work related problems. %3C/li%3E%0A%3Cli%3EAssists willingly in coverage of other patients. Performs insurance verification and re-verification. Assists with other departmental paperwork as needed. %3C/li%3E%0A%3Cli%3EParticipates in performance improvement activities as directed. Attends at least 80% of scheduled staff meetings and 100% of mandatory educational sessions.
