Description of Work
This position is responsible for leading team meetings and work groups of families of children and youth with special health care needs (CYSHCN) to work on the development and maintenance of effective programs for CYSHCN. The Family Liaison Specialist will be responsible for assisting families of CYSHCN with information and referrals through a toll-free Help Line for CYSHCN as requested. This position will assist in implementing and monitoring a contract with an external agency to provide timely reimbursements for family members. Additionally, the position will guide the Whole Child Health Section staff as to family needs and work with the development of family-centered programs. The position will also provide support to the Innovative Approaches Initiative, a systems change initiative focused on CYSCHN, as requested.This position trains, assists and provides guidance to staff on family related activities including, but not limited to, family perspectives, family centered care, care coordination, transition planning, medical home, and educational/community resources. The position staffs teams and work groups of families to assure Whole Child Health Section programs and services are family centered. Inclusion is a strong national priority of the Maternal and Child Health Bureau, our federal funding agency. The position will assure effective and ongoing communication between the Division/Section/Unit staff, other state and federal agencies, including the Maternal and Child Health Bureau, contractors and external local, state and national partners and organizations

Recruitment Range $45,431 - $60,141
**Position is eligible for a hybrid-teleworking schedule to include weekly onsite work hours**

The Knowledge, Skills, and Abilities (KSAs)/ Management Preferences are not required. Applicants who possess the following skills are preferred:
Strong first-hand working knowledge of, and experience working with, children and youth with special health needs (CYSHCN) and health care professionals who serve CYSCHN and their families.
Knowledge of educational and medical systems as they relate to CYSHCN, in addition to state and community-based services.
Experience in leading family/caregiver engagement efforts, such as family-focused committees and work groups and trainings/educational opportunities for families and providers.
Strong oral, written and project development skills.

Master’s degree in social work from an appropriately accredited institution; or Bachelor’s degree in social work from an appropriately accredited institution and one year of directly related experience; or
