About this role
Must hold a current social worker licensure (master's or bachelor's pathway), 2+ years community health experience, 1+ years working with behavioral health/SDoH and long-term care services, proficiency with electronic documentation and MS Office, reside in Michigan, valid driver’s license and reliable transportation, able to travel up to 75%.
