• Schedule: 4-10hr Shifts, Monday - Friday
• Department: Novant Health Heart and Vascular Institute
• Location: Charlotte Region - Float between Mooresville and Huntersville Clinics
• Certification: Will have to obtain the Heart Failure Certification within 1yr of hire

What We're Looking For:

• Education: 4 Year / Bachelors Degree BSN, preferred. 
• Experience: 3 years of experience in Telemetry, CCU, Cardiac Stepdown; Minimum of 3 years of Clinical RN experience, required.
• Licensure/Certification: Registered as RN in NC or SC, required. Certification as a Certified Heart Failure Nurse, required.
• Additional Skills Required:
  • Additional Skills/Requirements (required)
  • Demonstrates interdisciplinary leadership skills, superior interpersonal communication skills, and able to communicate patient’s needs to providers.
  • Organization skills, critical thinking skills, and clinical experience/expertise in HF management
  • • Ability to work independently on their own as well as collaborative with the interdisciplinary team.

What You'll Do:

• Collaborate with the Heart Failure team in all aspects of patient care, coordinating care needs of the heart failure population from the time of referral through end-of-life.
• Provide disease self-management education and serve as a resource for heart failure patients.
• Actively participate in performance improvement initiatives to enhance care, processes, and outcomes.
• Triage patient phone calls and collaborate with medical providers to titrate medications within scope of practice.
