Home Care Social Worker - Farmington

Shift Information:  Monday through Friday 8:00 AM – 4:30 PM

Home Office Location:  - 301 S. Jefferson St, Farmington, MO

Job Responsibilities:  - Meeting with patients/families, determine resource needs and making referrals, determine social support needs, education and planning for end-of-life needs, counseling support needs for anticipatory grief. 

Preferred skills or required work experience:  home health/hospice experience is not required, but a plus.  Additionally, this is a very independent position, prior social work experience is preferrable

The Parkland Intermittent Home Care Department of BJC Home Care Services provides home visits to patients in Farmington, Missouri and several nearby counties, with 24 hour on-call home care nursing supervision. Role Purpose

Under direction of a physician, assists patients and their families in their residences to adapt and plan an interdisciplinary approach to treating the patient-family system.
