What You’ll Do

• Complete start-of-care, recertification, and routine skilled nursing visits
• Perform comprehensive patient assessments in the home
• Develop and update plans of care based on patient needs
• Educate patients and families on disease management and self-care
• Monitor condition changes and communicate with physicians/team
• Supervise LVNs/LPNs and Home Health Aides as needed
• Document visits accurately and timely
• Ensure compliance with home health regulations and payer requirements

What You Bring

• Active RN license
• Valid driver’s license & auto insurance
• CPR certification
• Home Health experience strongly preferred
• Experience with HomeCare HomeBase is a plus
