Conducts initial and ongoing comprehensive assessments of home health patient needs, including OASIS assessments at appropriate time points.%3C/li%3E%3Cli%3EEvaluates outcomes of patient care and identifies discharge planning needs. Consults with other disciplines as required. Adjusts nursing care processes as necessary.%3C/li%3E%3Cli%3EMaintains patient records and charts to ensure accurate record of treatment, including med profile, care plan, communication notes, etc.%3C/li%3E%3Cli%3EProvides instructions to patients/families for use of products and services utilizing nursing process to address learning needs.%3C/li%3E%3Cli%3EPrepares, administers, monitors, and discontinues IV fluids.
