As a PRN Hospice Field RN, you’ll deliver compassionate, one-on-one care to patients facing life-limiting illnesses—primarily in the place they call home. You’ll have the autonomy to make clinical decisions in the field while being fully supported by a collaborative interdisciplinary team.

This role is ideal for an experienced hospice RN who values flexibility, independence, and deeply meaningful work.


What You’ll Do

  • Perform initial and ongoing comprehensive assessments of patients’ physical, functional, psychosocial, and environmental needs
  • Determine hospice eligibility, admission, and recertification using established clinical criteria
  • Develop, implement, and adjust individualized plans of care to ensure comfort and quality of life
  • Provide education and emotional support to patients and families on disease progression, end-of-life care, and self-care techniques
  • Coordinate with the interdisciplinary group (IDG) including physicians, social workers, and chaplains
  • Assess caregiver capability and identify support needs throughout the care journey
  • Supervise and provide clinical direction to Hospice Aides and LVN/LPNs
  • Document timely and accurate clinical and progress notes
  • Communicate changes in patient condition and collaborate on care adjustments

What You’ll Need

  • Active Registered Nurse (RN) license
  • CPR certification
  • Valid driver’s license and auto insurance
  • Minimum 1 year of hospice experience preferred (2+ strongly preferred)
  • Experience with Homecare Homebase is a plus
