Summary of Job:

The In-Home Support Associate-Per Diem will provide care and support to individuals with intellectual and/or developmental disabilities as needed. This role typically includes assisting with daily living activities, personal care, medication administration, and supporting individuals in community activities. The IHS Associate will contribute to their Individual Development Plans, encouraging progress and helping participants thrive at home and in the community. The IHS Associate must be patient, compassionate, and have a strong commitment to improving the quality of life for individuals with disabilities. The per diem aspect means the IHS Associate works on an on-call or irregular schedule, rather than full-time or part-time with set hours.

Skills and Requirements:

  • High School diploma or equivalent. Additional training or certification in a related field is a plus
  • Previous experience working with individuals with disabilities is preferred
  • Strong interpersonal and communication skills, patience, and a compassionate nature
  • Valid driver's license and reliable transportation
  • May be required to transport individuals in personal vehicles
  • CPR, First Aid, and other relevant certifications (or willingness to obtain)
  • Must pass a criminal background check and any other required screenings
  • Flexibility in scheduling and ability to work on short notice
  • Ability to work per diem shifts, including weekends, evenings, and holidays as needed
  • Work is primarily performed in individuals' homes and community settings


Position Duties and Responsibilities 

  • When needed, assist individuals with daily living activities, including bathing, dressing, grooming, and toileting

  • Administer medications as prescribed, monitor health conditions, and ensure a safe living environment
  • Support individuals in developing and maintaining skills for independence, such as cooking, cleaning, and budgeting.
  • Encourage participation in community activities, social events, and recreational opportunities
  • Implement individualized support plans and positive behavior interventions
  • Maintain accurate and timely service records, including daily logs, incident reports, and progress notes
  • Advocate for the rights and needs of individuals, ensuring they receive appropriate services and support
  • Maintain open and effective communication with individuals' families, supervisors, and other team members 
  • Monitor individuals physical and emotional well-being and report any changes, concerns, or incidents
  • Follow all Easterseals policies, procedures, and safety protocols, including emergency response procedures
  • Maintain professional boundaries and confidentiality in accordance with organizational standards and applicable regulations
  • Utilize approved strategies to support individuals with behavioral challenges, including de-escalation techniques when needed
  • Ensure all documentation is completed accurately, timely, and in compliance with program and regulatory requirements
  • Demonstrate reliability and flexibility in accepting and working assigned per diem shifts
  • Perform other duties as assigned.

Physical Demands Efforts 

This function requires moderate standing, walking, bending, squatting, driving and sitting. Position requires ability to safely lift, transfer, and reposition participants with assistance or use of assistive equipment and/or adaptive equipment, and monitor condition of equipment. Requires lifting and moderate carrying of items up to 25 pounds. Ability to operate normal office equipment such as telephone, copy machine, fax, scanner, and computer.

Specific vision abilities required by this job include close vision, distance vision and ability to adjust focus. 

Work Environment and Hazards

This work is primarily performed in individuals' homes and community settings, as such, work environments will vary.
