Home Health Intake Coordinator (Medical Assistant / CNA / LVN)

The Home Health Intake Coordinator serves as the primary point of contact for new patient referrals, ensuring a smooth transition into home health services. This role is responsible for managing referrals, verifying insurance coverage, obtaining authorizations, coordinating with clinical teams, and maintaining accurate patient records within the EMR system.

Key Responsibilities

• Receive and process new patient referrals in a timely manner.
• Verify insurance benefits, authorizations, and certifications.
• Coordinate patient admissions with nursing supervisors and interdisciplinary teams.
• Communicate effectively with referral sources, patients, families, and healthcare providers.
• Maintain accurate documentation and data entry in EMR systems.
• Support quality improvement initiatives and agency compliance standards.

Qualifications

• Medical Assistant, CNA, or LVN/LPN required 
• At least 1 year of referral intake experience preferred.
• Experience with Medicare, Medicaid, and insurance verification highly preferred.
• Knowledge of medical terminology and healthcare processes.
• Strong computer, customer service, and communication skills.
• HomeCare HomeBase experience preferred.
• Valid Driver's License and Auto Insurance.
