Required Qualifications: 

• Possess one of the following:
  • Indiana licensed Registered Nurse in good standing with 3+ years of case management experience
  • Indiana Master’s Social Worker with 3+ years of case management experience
  • Bachelor’s and/or Master’s degree in Social Work, Gerontology, Counseling, Psychology, or Nursing with a minimum of 2 years full time direct service experience with the elderly or disabled which includes assessment, care plan development and monitoring
• 2+ years of leadership experience with responsibility for team performance OR 3+ years of experience as a UnitedHealthcare care coordinator/case manager with experience serving as a peer mentor and/or coach
• 2+ years of experience providing support or care coordination for older adults
• 2+ years of experience working within the community health setting in a health care role
• 2+ years of experience facilitating and working in coordination with an interdisciplinary team
• Intermediate level of proficiency with MS Office skills, including Word, Excel, Outlook, and PowerPoint
• Resident of Indiana
• Access to reliable transportation and valid US driver’s license with the ability to travel within assigned territory to meet with employees, members and providers

Preferred Qualifications:

• Experience with Long Term Service and Supports or Waivers Program
• Management experience in managed care setting
• Proven ability to navigate a TEAMs environment

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). No matter where or when you begin a career with us, you’ll find a far-reaching choice of benefits and incentives. The salary for this role will range from $91,700 - $163,700 annually based on full-time employment. We comply with all minimum wage laws as applicable.
