Qualifications:

• Bachelors Degree in Nursing and Michigan Registered Nursing License (RN)
• One year of experience in home health care, mental health nursing or hospital
  setting.
• Strong clinical and communication skills.
• Ability to travel within the service area, provide valid driver’s license and
  automobile insurance.
• Ability to exercise independent judgement

Duties and Responsibilities

1. Collaborates as part of an interdisciplinary team in assessing, planning, implementing, and evaluating services for psychiatric, co-morbid and/or co-occurring psychiatric and substance use disorder consumers.
2. Completes nursing assessments to determine the needs of individuals for medical assistance, takes clinical histories and refers individuals to a physician/pediatrician/specialty physician when appropriate.  
3. Monitors assessments and screenings to assure findings are integrated in the care plan.
4. Participates in initial care plan development including specific goals for all enrollees.
5. Performs ongoing assessments and updates to care plans as needed.
6. Participates in the selection of strategies to implement evidence-based wellness and prevention initiatives.
7. Coordinates and communicates with medical providers, subspeciality providers, long term care and hospitals regarding records including admission/discharge.
