Job Title: Occupational Therapist (PRN) – Home Health Care

Job Type: PRN (As Needed)

Position Summary:
We are seeking a compassionate and skilled Occupational Therapist (PRN) to join our home health care team. In this role, you will provide high-quality, patient-centered therapy services to individuals in their homes, helping them regain independence and improve their quality of life. This position offers flexibility and the opportunity to make a meaningful impact within the community.

Key Responsibilities:

• Evaluate patients’ functional status and develop individualized treatment plans

• Provide therapeutic interventions to improve daily living and mobility skills

• Educate patients, families, and caregivers on adaptive techniques and equipment

• Assess home environments and recommend modifications for safety and accessibility

• Maintain accurate and timely documentation in compliance with regulations

• Collaborate with interdisciplinary team members to coordinate patient care

• Monitor patient progress and adjust treatment plans as needed

Qualifications:

• Current, active Occupational Therapist license in Ohio

• Graduate of an accredited Occupational Therapy program

• Minimum of one (1) year of clinical experience preferred home health experience a plus but we do accept and coach new grads in the field

• Strong clinical assessment and critical thinking skills

• Excellent communication and interpersonal abilities

• Ability to work independently and manage time effectively

• Current CPR certification

• Valid driver’s license and reliable transportation
