Job Title: Occupational Therapist (PRN) – Home Health Care
Location: Lorain County
Job Type: PRN (As Needed)
Position Summary:
We are seeking a compassionate and skilled Occupational Therapist (PRN) to join our home health care team. In this role, you will provide high-quality, patient-centered therapy services to individuals in their homes, helping them regain independence and improve their quality of life. This position offers flexibility and the opportunity to make a meaningful impact within the community.
Key Responsibilities:
Evaluate patients’ functional status and develop individualized treatment plans
Provide therapeutic interventions to improve daily living and mobility skills
Educate patients, families, and caregivers on adaptive techniques and equipment
Assess home environments and recommend modifications for safety and accessibility
Maintain accurate and timely documentation in compliance with regulations
Collaborate with interdisciplinary team members to coordinate patient care
Monitor patient progress and adjust treatment plans as needed
Qualifications:
Current, active Occupational Therapist license in Ohio
Graduate of an accredited Occupational Therapy program
Minimum of one (1) year of clinical experience preferred home health experience a plus but we do accept and coach new grads in the field
Strong clinical assessment and critical thinking skills
Excellent communication and interpersonal abilities
Ability to work independently and manage time effectively
Current CPR certification
Valid driver’s license and reliable transportation
