Position Summary ORTHO/NEURO NURSE NAVIGATOR South Lake Hospital serving Jewett Orthopedic Institute Full-Time Position Summary This role will support the orthopedic patient population at South Lake Hospital. The growing Varma surgical group brings total joint knee, hip, and shoulder patients that will need the support of a Nurse Navigator for pre and postoperative follow up in line with the work being done central,east, and west. This position will follow the same structure as the already established Ortho/neuro nurse navigator role. Education/Training An Associate of Science in Nursing (ASN) is required. A Bachelor of Science in Nursing (BSN) is preferred. Licensure/Certification • Maintains license as an RN in the state of Florida. • Maintains current BLS/healthcare provider certification. Experience Minimum of two (2) years of clinical experience in acute care is required. Ortho/Neuro experience is preferred. Essential Functions • Coordinates patient care through the continuum. Assists with timely access to care, coordinates/facilitates appointments and services, and encourages adherence to treatment plans, protocols, and follow up care. • Assesses patients’ current and future needs, provides access to resources (internal and external), and makes appropriate referrals as needed. • Provides education and information to patients and families related to the healthcare system, multidisciplinary team member roles, plan of care, and available resources. • Establishes and maintains close collaboration with physicians and multidisciplinary team members to ensure seamless patient care and care coordination. • Tracks determined program metrics and quality indicators. Documents patient interactions and progression, and communicates navigation activities to physicians, the care team, and leadership. • Collaborates with the clinical team to obtain or develop patient and family educational materials and resources. • Demonstrates knowledge, skill, and coordination to provide nursing care and guidance to patients from screening to post-operative care and follow up. • Demonstrates effective communication with patients, families, peers, members of the multidisciplinary healthcare team, and representatives of community organizations and resources. • Maintains reasonably regular, punctual attendance consistent with Orlando Health policies, the ADA, FMLA and other federal, state and local standards. • Maintains compliance with all Orlando Health policies and procedures
