If you are looking for an exciting and challenging opportunity, join the team of Virginia Mason Franciscan Health Rehabilitation Hospital. This 60- Bed Hospital serves patients recovering from strokes, brain and spinal-cord injuries, amputations, complex orthopedic injuries, and other conditions.

How you’ll contribute

The Per Diem Outcomes Manager provides coordination of the timely and accurate data completion of the Inpatient Rehabilitation Facility-Patient Assessment Instrument (IRF-PAI) for each rehabilitation inpatient.  

• Collects, reviews and monitors the information contained in the IRF-PAI, on each patient to assure accuracy in the scoring of functional status and reflection status and reflection of the patient’s care needs, as documented in the medical record and the UDS Pro-Doc system, in collaboration with the interdisciplinary team.  
• Assists in the ongoing monitoring of identified outcome benchmarks for the inpatient population.  
• Access to confidential patient information and maintains all confidentiality of all information. 

What we're looking for 

Applicants should have a Bachelor of Science in Nursing, Associate Degree in Nursing or Nursing Diploma program or Licensed/Registered PT, OT, SLP from an accredited school. 

Additional requirements include:

• Required: Current state licensures such as Registered Nurse (RN), Physical Therapist (PT), Occupational Therapist (OT) or Speech Language Pathologist (SLP). 
• Required: CPR/BCLS certification.
• Preferred: 3 to 5 years of experience in the care of patients in the acute hospital, skilled nursing facility or rehabilitation setting.
• Preferred: Excellent oral and written communication and interpersonal skills. Ability to work independently. 
• Preferred: Flexibility with working up to 20 hours per week, including some weekends.
