Position Duties
PH RN 1:
Under supervision, provides professional nursing care in accordance with nurse protocols, policies and/or procedures
PH RN 2:
Under general supervision, provides professional nursing care in accordance with nurse protocols, policies and/or procedures. Focuses on the prevention of illness, injury or disability, the promotion of health and maintenance of the health of populations. Cares for population with special needs during disasters/emergencies and biological and chemical protocols.
Additional Duties:
- Provide outreach, education, and case management to high-risk perinatal & postpartum women, and infants up to one year of age through home visits.
- Conduct home visits for prenatal, postpartum, and infant assessments to assess patient conditions and coordinate with providers to ensure appropriate care.
- Educate patients, offer referrals for additional services, and document observations and follow up plans.
- Attend required meetings (local, District, and State).
- May supervise additional staff as needed.
- Willing to travel within District 2 as necessary.
- Familiar with OB/GYN hospital practices
- Other duties as assigned
Minimum Qualifications
PH RN 1:
Associate's degree in nursing from an accredited college or university AND completion of a PHN course within one year of hire AND current Georgia Registered Professional Nurse license AND CPR certification. Note: No experience required.
PH RN 2:
Bachelor's degree in Nursing from an accredited college or university AND Current Georgia license to practice as a Registered Professional Nurse AND CPR certification OR
Associate's degree/diploma in Nursing from an accredited college AND One year of PHN experience AND Completion of a PHN course within one year of hire AND Current Georgia Registered Professional Nurse license AND CPR certification.

- Superior interpersonal skills
- Experience with working with prenatal, postpartum women and infants
- Ability to work efficiently under established guidelines
- Excellent written and oral communication skills
- Ability to communicate effectively with corporate clients
- Self-directed & self-motivated
- Ability to complete tasks without detailed supervision
- Excellent computer skills
- Ability to read, write and speak Spanish
