About this role
Must hold Wisconsin and Minnesota pharmacist licenses and a PharmD (or BS in Pharmacy with ≥10 years relevant experience). MTM certificate required within 12 months if limited MTM experience; MHCP registration required. Ambulatory care, PGY-2, rheumatology experience and BCACP preferred.
