Primary Responsibilities:

  • Provide physical therapy services to patient according to a written physician’s plan of care. This may include, but will not be limited to:
    • Assessing and evaluating therapeutic/rehabilitative/functional status, participation in the development of the total plan of care and evaluates home environment
    • Assesses for muscle strength, mobility, gait, and range of motion - potential for rehab
    • Directing physical therapy treatment
    • Instructing patients and/or families/caregivers in the use and care of therapeutic appliances.
    • Determining priority needs for physical therapy
    • Reports to physician patient’s reaction to treatment or changes in condition
  • Initiates physical therapy program and instructs other personnel and/or family caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences
  • May train patient in the use of prosthetic device and/or orthotic devices
  • Identifies patient and family/caregiver needs for other home health services and refers as appropriate
  • Prepares and submits clinical and progress summaries based on the attainment of goals
  • Participates in discharge planning for patient
  • Provides physical therapy consultation to families/caregivers when indicated
  • Provide in-service education related to Physical Therapy to Agency personnel as needed
  • Participates in peer consultation process
  • Supervises physical therapy assistants according to organization policy and state regulations, treat and re-evaluation patient no less than every 7th day of treatment or 21 days whichever occurs first
  • Maintains contact/communication with other personnel involved in the patient’s care to promote coordinated, efficient care
  • Maintain clinical competency in physical therapy practice and theory
  • Will participate in Agency on-call coverage as outlined in Agency policy

Required Qualifications:

  • Degree from a baccalaureate or master’s program or doctorate in physical therapy approved by an accredited organization
  • Active unrestricted State of Nevada license to practice as a Physical Therapist
  • Have and maintain current CPR certification
  • Experience working with an electronic medical record application
  • Demonstrates thorough knowledge of current Physical Therapy professional standards of care, and state and federal regulations
  • Basic PC skills to include Window, Outlook and Other email systems
  • Valid Nevada driver's license with automobile that is in good working order and maintain personal auto insurance coverage in accordance with organization requirements

     

 

Preferred Qualifications:

  • 1+ years of appropriate experience as a Physical Therapist
  • Community/home health experience Demonstrate excellent observation, verbal and written communication and organization skills
  • Complies with accepted professional standards and practices
  • Proven ability to prioritize and communicate objectives clearly

 

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $72,800 - $130,000 annually based on full-time employment. We comply with all minimum wage laws as applicable.
