Responsibilities
- Evaluate patients and develop individualized, evidence-based treatment plans to improve mobility, function, and quality of life.
- Perform comprehensive assessments and establish functional goals based on each patient's needs and diagnosis.
- Deliver skilled therapeutic interventions while monitoring progress and modifying treatment plans to achieve optimal outcomes.
- Collaborate with patients, families, providers, and interdisciplinary team members to coordinate high-quality, patient-centered care.
- Educate patients and caregivers on diagnoses, treatment plans, home exercise programs, injury prevention, and wellness strategies.
- Determine when referrals or additional services are appropriate to support the patient's plan of care.
- Supervise and delegate appropriate patient care activities to Physical Therapist Assistants, therapy technicians, and students.
- Participate in care conferences, quality improvement initiatives, and program development to enhance patient outcomes and the therapy experience.
- Maintain accurate, timely documentation and comply with regulatory, licensure, and payer requirements.
- Pursue ongoing professional development while contributing to a collaborative, supportive rehabilitation team.
Qualifications
- Graduation from an accredited Physcial Therapy Program
- If program graduation is prior to 2005, a Bachelor's degree is required
- If program graduation is from 2005-2019, a Master's degree is required
- If program graudation is 2020-prsent, a Clinical Doctorate (not PHD) is required
- Current Physical Therapist license in the state of practice, or eligible to obtain licensure prior to start.
- Previous outpatient or related Physical Therapy experience preferred; new graduates are encouraged to apply.
- Strong clinical assessment, critical thinking, and patient education skills.
- Excellent communication and collaboration skills with patients, families, providers, and interdisciplinary team members.
- Ability to develop and implement individualized, evidence-based treatment plans.
- Commitment to delivering compassionate, patient-centered care and ongoing professional growth.
