Speech Language Pathologist Home Care

Job Summary

We are seeking a compassionate and motivated Speech Language Pathologist (SLP) to join our Home Care team on a part-time basis. The SLP will provide speech, language, cognitive, and swallowing evaluations and treatments to patients in their homes, helping them improve communication, safety, and overall quality of life.

Responsibilities

• Evaluate patients with speech, language, cognitive, voice, and swallowing disorders

• Develop individualized treatment plans based on patient needs and physician orders

• Provide skilled speech therapy services in the patient’s home environment

• Educate patients, families, and caregivers on treatment techniques and home exercise programs

• Maintain accurate and timely clinical documentation

• Collaborate with physicians, nurses, therapists, and interdisciplinary team members

• Monitor patient progress and adjust treatment plans as needed

• Comply with all agency policies, professional standards, and regulatory requirements

Qualifications

• Master’s Degree in Speech Language Pathology

• Current state licensure as a Speech Language Pathologist

• Certificate of Clinical Competence (CCC-SLP)

• Current CPR certification preferred

• Previous home health or geriatric experience is a plus

• Excellent communication and interpersonal skills

• Reliable transportation and valid driver’s license
