Position Duties
This position focuses on assessing needs, coordinating services, and supporting older adults and eligible individuals through clear guidance and responsible care management. Work involves maintaining cases, completing assessments, and assisting consumers with information about available programs. As an Aging Care Manager 2, you will perform the following duties:
- Care Assessment: Complete intake and reassessment duties and document information in the appropriate systems
- Service Coordination: Arrange needed services and work with community partners to support consumer needs
- Program Guidance: Explain program eligibility, limits, and procedures to consumers and families
- Case Maintenance: Manage caseload tasks such as reports, records, and required follow-up contact
- Information Review: Interpret requests from professionals and determine appropriate referrals
Work Schedule and Additional Information:
- Full-time employment
- Work hours are 8:00 AM to 4:30 PM, Monday - Friday, with a 30-minute lunch.
- On-call hours will be required
- Overnight or out of county travel for meetings or training will be required
- Telework: You will not have the option to telework in this position.
- Salary: In some cases, the starting salary may be non-negotiable.

Minimum Qualifications
QUALIFICATIONS
Minimum Experience and Training Requirements:
- Six months as an Aging Care Manager 1 or a County Caseworker 1; or
- Successful completion of the County Social Casework Intern program; or
- Six months of professional human services or nursing experience; and a bachelor’s degree in social sciences, behavioral sciences, human services, or a closely related field; or
- An equivalent combination of experience and training.

Legal Requirements:
- You must pass a background investigation.
- A conditional offer of employment will require a drug screening.
- This position falls under the provisions of the Child Protective Services Law.
- Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
