About this role
Must be qualified to independently complete assessments within scope of practice (RN, SW, OT, PT, ST, Rehabilitation Counseling); active license if required; CCM or ACM certification required or within 2 years; minimum Associate Degree in Nursing or Bachelor's for other disciplines; 2 years rehab experience preferred.
