The Rural Health Transformation (RHT) Center for Rural Health Sustainability & Innovation (CRHSI) Program Manager will:

• Support provider-facing technical assistance, operational coordination, and implementation support activities associated with the Center for Rural Health Sustainability & Innovation (CRHSI) within the New Mexico Rural Health Transformation (RHT) Program;
• Coordinate with rural providers, hospitals, clinics, contractors, technology partners, state agencies, and external stakeholders to support operational improvement, implementation planning, provider readiness, and sustainability-focused activities;
• Support implementation of the RHT governance structure by coordinating with providers, contractors, state agencies, and external stakeholders to strengthen rural provider sustainability, operational performance, and implementation readiness across RHT initiatives.

The RHT CRHSI Program Manager is responsible for:

• Leading provider-facing technical assistance and systems support activities for CRHSI and the RHT Program;
• Supporting rural providers, health systems, clinics, and community partners through operational technical assistance, systems support, implementation coordination, and provider engagement activities aligned with the State's rural health transformation goals;
• Supporting providers with technical assistance related to operational workflows, implementation barriers, care delivery coordination, financial sustainability, technology adoption, workforce challenges, and shared service opportunities;
• Facilitating provider engagement meetings, learning collaboratives, implementation discussions, and operational coordination activities;
• Monitoring provider implementation progress, operational risks, technical assistance needs, and escalation issues across participating providers and communities;
• Developing provider-facing materials, implementation tools, operational guidance, executive summaries, stakeholder communications, and tracking tools;
• Collaborating with Program Management Office (PMO) leadership, CRHSI leadership, operations, communications, data and evaluation, procurement, and finance staff to support aligned implementation and technical assistance across the RHT Program portfolio.

• Work is performed for the Health Care Authority (HCA), the RHT Program Management Office, CRHSI, rural providers and health systems, contractors, technology partners, state agencies, CMS, and program stakeholders to support provider sustainability and implementation of the Rural Health Transformation Program.

The Ideal Candidate has:

• Experience supporting rural health, health care operations, technical assistance, systems implementation, provider engagement, practice transformation, or program implementation activities within a public sector, Medicaid, public health, or health care environment;
• Strong communication, relationship-building, operational coordination, facilitation, and problem-solving skills, with experience supporting providers through operational improvement or implementation activities;
• Experience working with rural hospitals, clinics, FQHCs, Tribal health organizations, or community-based providers;
• The ability to demonstrate adaptability, professionalism, attention to detail, sound judgment, and the ability to manage multiple priorities simultaneously within a fast-paced implementation environment.
