The Chronic Care Nurse Coordinator at Montefiore Medical Group plays a pivotal role in enhancing the quality of care for patients with chronic illnesses. Responsible for overseeing the comprehensive management of patients with long-term health conditions, the coordinator collaborates with healthcare teams to develop and implement individualized care plans. Duties include conducting thorough patient assessments, educating individuals and their families on disease management strategies, coordinating regular follow-up appointments, and ensuring adherence to treatment plans. The role involves close collaboration with physicians, nurses, and other healthcare professionals to optimize patient outcomes. Additionally, the Chronic Care Nurse Coordinator actively engages in patient advocacy, facilitates communication between various healthcare providers, and stays abreast of the latest advancements in chronic care management. This position demands strong clinical expertise, excellent communication skills, and a compassionate approach to support patients in achieving optimal health outcomes.

Shift: Monday to Friday 0900A – 0530P

Responsibilities

  • Contributes to the actualization of the vision philosophy and critical success factors of the Department of Nursing
  • Commits to maintain the standard of the practice of nursing and are responsible for the coordination, implementation, and delivery of care to the neonate, pediatrics, adolescent, adult and/or elderly patients within their scope of practice
  • Participates in performance improvement, research activities and maintains clinical expertise
  • Coordinates the continuum of medical care services for patients to ensure optimum utilization of resources, delivery of quality patientcare, and compliance with regulatory, accrediting, and contractual requirements 
  • Determines the appropriateness of patient care services, prepares clinical appeals for third party denials, and collects and interprets data for utilization, compliance, QI, and other areas as assigned

 
Requirements

  • NY State License for Registered Nurse and current registration
  • BSN Required, MSN preferred
  • Certification in specialty preferred
  • Effective English communication skills, written, oral, and interpersonal
  • Ability to use basic Excel data entry and retrieval system
