Temporary license is acceptable during first 90 days of employment Upon Hire%3Cbr%3E%26nbsp;%3C/li%3E%3C/ul%3E%3Cp%3E%3Cstrong%3EPreferred%3C/strong%3E%3C/p%3E%3Cul%3E%3Cli%3E%3Cstrong%3EExperience%3C/strong%3E%3Cbr%3E2 years related experience.%26nbsp;%3Cbr%3E%26nbsp;%3C/li%3E%3C/ul%3E%3Cp%3E%3Cbr%3E%3Cstrong%3EJOB DUTIES%3C/strong%3E%3C/p%3E%3Cul%3E%3Cli%3EParticipates in multidisciplinary rounds and proactively determines patient needs for discharge transitions including transportation, DME, LTAC, SNF, home health agencies and hospice.%3C/li%3E%3Cli%3EUtilizes crisis intervention and problem solving skills to facilitate and enhance patient and family%27s ability to meet acute and ongoing medical and psychosocial needs.%3C/li%3E%3Cli%3EDemonstrates ongoing familiarity with community resources to facilitate positive patient and family outcomes agencies to assure appropriate client access.%3C/li%3E%3Cli%3EMaintains effective documentation including tracking and referral information; consistently meets department and UTSW documentation standards.%3C/li%3E%3Cli%3EIdentifies and reports situations or cases to appropriate governmental agencies as legally required and in compliance with organizational policies and procedures.%3C/li%3E%3Cli%3EServes as an active participant of the health care team, communicating effectively with other health care providers, patients, families, other hospital personnel, and visitors.%3C/li%3E%3Cli%3EIdentifies the need for professional growth and seeks appropriate development opportunities.%3C/li%3E%3Cli%3EProvides education to staff, patients and community resource affiliates.%3C/li%3E%3Cli%3EMaintains system for tracking placement of patients after discharge on monthly basis.%3C/li%3E%3Cli%3EHas knowledge of Department and Unit Performance Improvement program and is aware of expected participation.
