POSITION SUMMARY:
Under the supervision of the RN Case Manager or Director of Case Management, the LVN Discharge Planner works collaboratively with physicians, nursing staff, patients, families, and community partners to develop and implement individualized discharge plans. The LVN Discharge Planner plays a vital role in the coordination of post-acute care services to ensure safe and effective discharges and continuity of care. This role requires knowledge of state and federal
guidelines and criteria for determining medical necessity, level of care, resource utilization, and continued length of stay. Clinical knowledge of signs, symptoms, treatments, therapies, expected outcomes, and discharge needs for patients of all age groups is essential. This is a non-union role.
ESSENTIAL JOB FUNCTIONS
- Identify discharge needs through patient interviews and chart reviews
- Initiate and coordinate referrals for:
- Durable Medical Equipment (DME)
- Home Health (including coordination of home health discharges)
- Skilled Nursing Facilities (SNF)
- Ensure discharge plans align with medical condition, insurance coverage, and patient preferences
- Assist with patient education related to post-discharge care
- Document all discharge planning activities accurately and timely
- Maintain follow-up communications with patients and post-acute providers as needed
- Support hospital quality initiatives, including readmission reduction
- Adhere to all safety protocols and procedures
- Alternating weekly schedule:
- Availability to work weekends and holidays as needed
- Must be able to work in all patient care and operational areas, with reasonable accommodations as applicable
- The LVN also assists with patient education, documentation, and follow-up communications as part of the hospital’s quality and readmission reduction efforts.
Job Details: 10-hour shifts
Qualifications
Education:
- Completion of clinical training in an accredited LVN program
Experience:
- Previous clinical experience must enable the applicant to pass required clinical competencies prior to hire.
Licenses/Certifications:
- Current California Licensed Vocational Nurse
- Board of Vocational Nursing Intravenous Certification or acquire within six months in the position
- Current BLS Certification through AHA is required.
