About the Department
The West Virginia Veterans Nursing Facility is seeking compassionate and dedicated Registered Nurses to join our long-term care team.
You can make a difference in the lives of Veterans by Serving Those Who Served!As a Registered Nurse in a long-term care setting, your duties are focused on managing chronic conditions and providing continuous care. You are responsible for conducting regular health assessments, monitoring vital signs, and observing residents for any changes in their medical or cognitive status. Your key responsibilities will include:
Direction and supervision of all resident care, including administering and documenting resident care, medication, and treatments per physician's order.
Provide instruction and supervision of licensed practical nurses and certified nursing assistants.
Promptly report changes in residents' condition to the physician and take follow up action as necessary.
Follow facility admission procedures and communicate resident admissions to other departments.
Take appropriate action to assure adequate staffing to meet resident needs.
Identify resident problems and emergency situations and initiate emergency care.
Primary Duties include completing MDS's as required (admission, quarterly, annual, significant change); complete admission care plan based on resident current needs and treatments; update care plans with new orders as needed; review and update care plans quarterly and with significant changes; completes assessment quarterly, annually and with significant changes; hold care plan meetings between Inter-Disciplinary Team and families quarterly, annually and with significant changes as needed; complete psychoactive medication consent forms.
Administrative Nurse Duties include: take turn working floor when supervisor has called off or there is not a supervisor scheduled; complete audits on designated hallway when designated nurse is not available; complete audits as assigned; attend QA meetings; participate in development of policies and procedures' complete write-ups for staff as needed; complete investigations and complaints as assigned; assist in state and federal surveys; follow-up with new hires to discuss how orientation/position is going; interview potential staff when needed; cover DON/ADON as needed when needed; make rounds on the floor to monitor staff; all other duties as assignedThis position is full time (40 hours per week), with day and night shifts available. Must have flexibility to cover shifts as needed, including nights, weekends and holidays to ensure continuous, safe staffing levels.

Minimum Qualifications
Minimum Qualifications:
ASN or BSN from an accredited college or university and 1 year of full time or equivalent part time paid experience as a licensed practical nurse or registered nurse.
Special requirement: Current West Virginia registered nurse license.
