Required Qualifications:
Current, unencumbered license to practice physical therapy in the assigned state
Graduate of an accredited physical therapy program (CAPTE accredited)
1+ year of experience in physical therapy; OR less than one (1) year with participation in an approved residency or mentorship program
Current CPR certification
1+ year of experience in clinical assessment, documentation, and care coordination
Driver’s License and access to reliable transportation
Preferred Qualifications:
1+ year of experience in home health, hospital-at-home, or community-based therapy settings
1+ year of experience working within interdisciplinary healthcare teams
