Practice Transformation Manager

Job Description

Integra Connect is seeking an experienced clinical Practice Transformation Manager (PTM) to drive improvements in oncology practice performance within value-based care arrangements. This role requires a blend of data analysis and trend identification, clinical expertise, and strong consultative skills. The PTM will leverage claims and utilization management data from dashboards and reports to identify oncology practice workflow gaps and performance trends, collaborate with payor clinical experts on improvement strategies, and actively research, curate, and develop clinical content and best practices. They will then consult directly with oncology practices to implement these strategies, enhancing quality and efficiency aligned with value-based care goals, including generating cost savings and improving the overall quality of care for oncology patients.

Responsibilities:

• Facilitate the completion of a Practice Transformation Baseline Assessment by oncology practices and collate results to assist practices in identifying potential improvement opportunities in current state processes and structures.
• Utilize dashboards, reports, and underlying data to identify key trends in practice performance related to quality metrics, cost, and utilization within value-based care arrangements.
• Analyze data to identify gaps and inefficiencies in oncology practice processes, structures, and workflows that impact value-based care outcomes.
• Conduct in-depth analysis (“deep dives”) on practice performance data, particularly when practices are underperforming in specific quality, cost, and utilization areas.
• Collaborate effectively with clinical partners to create presentation materials (e.g., PowerPoint decks) to summarize real-time data insights, identify workflow gaps, and consult on potential improvement recommendations.
• Research and curate evidence-based clinical content, workflows, and best practices aligned to potential gaps in oncology practice processes and structures.
• When applicable, create new clinical content, workflow diagrams, and guides tailored to specific practice needs.
• Organize and maintain a resource library of all curated and created clinical content and best practices for easy access by the Practice Transformation Team and the oncology practices.
• Translate data insights and best practice expertise into clear and actionable strategies and workflow recommendations for oncology practices.
• Serve as a subject matter expert and consultant to oncology practices for value-based care requirements, quality improvement methodologies, and clinical workflow optimization.
• Conduct and participate in Joint Operating Council (JOC) meetings with participating practices on a regular cadence.
• Work with data teams and track whether the implementation of recommended workflows or use of curated content leads to measurable improvements in key performance indicators.
• Effectively collaborate with internal data analytics, reporting, and product teams to understand available data and reporting capabilities and provide feedback on dashboard and reporting enhancements, where applicable.
• Collaborate with internal and external teams to create and continuously update oncology practice risk profiles based on qualitative and quantitative data.
• Continuously track and monitor practice improvement activities against value-based care performance targets and provide feedback to practices on a regular cadence.
• Under the direction of their manager, prepare and distribute value-based care performance analysis and reports to various stakeholders, including payor clients, internal teams, external practices, and senior leadership.
• Maintain a strong understanding of value-based care models, quality measures, relevant clinical practices, and quality standards.

Qualifications

• 5+ years of experience in healthcare, with 2+ years specifically in value-based care, practice transformation, or healthcare quality improvement.
• Clinical background required (e.g., Registered Nurse (RN), Pharmacist (PharmD), Nurse Practitioner (NP), Physician Assistant (PA), Licensed Clinical Social Worker (LCSW), or other relevant licensed clinician). 
• Demonstrated experience directly building or supporting value-based care programs (e.g., ACOs, bundled payments, pay-for-performance). 
• Experience as a Quality Improvement Specialist or significant involvement in healthcare quality improvement initiatives. 
• Strong understanding of clinical processes and workflows in a community practice or health system setting. 
• Skilled at researching and collating clinical information, including best practices, workflows, and clinical guides. 
• Excellent communication, presentation, and interpersonal skills, with the ability to effectively collaborate and consult with clinical and administrative staff (including external vendors and payors). 
• Ability to translate data insights and clinical evidence into actionable, applicable strategies for workflow and process changes at a practice-specific level. 
• Demonstrated ability to lead consultative meetings and build strong relationships with external practice and payor stakeholders. 
• Bachelor’s degree in Nursing, Pharmacy, Healthcare Administration, Licensed Clinical Social Worker or a related healthcare field.
• Travel 10% is required.

Desired Qualifications:

• Certifications a plus: CPHQ, NCQA PCMH CCE, Lean Six Sigma, CCM.
• Clinical oncology experience or experience working directly with oncology practices. 
• Experience with Palliative Care and/or Hospice.
• Experience working with payors or health plans. 
• Experience with oncology focused value-based care models. 
• Experience working in a start up environment.
• Familiarity with common quality reporting frameworks (e.g., HEDIS, MIPS, OCM measures). 
• Experience using PowerBI and Tableau healthcare dashboards.
