Job Profile
Job Summary
The Patient Flow Command Center (PFCC) Nurse is responsible for coordinating and completing interfacility patient transfers across Harris Healths multi-hospital system as well as from external healthcare facilities, ensuring safe, timely, and appropriate patient placement. Expertise includes intake, prioritization, and coordination of transfer requests, facilitation of physician-to-physician communication, and collaboration with PFCC coordinators for bed management, and clinical teams to support effective patient flow, capacity management, and continuity of care. Demonstrate ability to work with internal departments and external facilities to ensure efficient patient movement, patient safety, and regulatory compliance in the role of a Registered Nurse. This position is also responsible for the maintenance of department records, accurate documentation into Epic, and any required data associated with transfer requests. This position will also be responsible for coordinating with PFCC Coordinators and all bed management within Harris Health to determine the necessity of transferring patients outside of the system.
Minimum Qualifications
Degrees:
Bachelor of Science in Nursing
Licenses & Certifications:
Certificate In a clinical or related specialty to be completed within two (2) years of hire. Other certificates as required by unit/service
Basic Life Support American Heart Association (AHA) or American Red Cross (ARC)
approved program
Registered Nurse State of Texas or valid multistate/compact license
Work Experience:
One (1) Year Work Experience Transfer Center, Bed Management, Case Management,
Quality Management, Coding or Discharge Planning. (Preferred)
Five (5) Years Work Experience as a Registered Nurse
Management Experience:
Two (2) Years of Management Experience Leadership or supervisory experience
Communication Skills:
Above Average Verbal Communication (Heavy Public Contact)
