Registered Nurse - Acute Care Case Manager (RN)

Great healthcare doesn't happen by accident—it happens through coordination, advocacy, and planning. 

As an RN Case Manager, you'll be the driving force behind a patient's journey through the healthcare system, ensuring they receive the right care, at the right time, in the right place. Working closely with physicians, nurses, therapists, social workers, patients, and families, you'll help remove barriers, coordinate services, and create safe, successful transitions throughout the continuum of care.

Requirements 

• Active Indiana/Kentucky RN license or valid compact RN license 
• 2 years of acute care RN experience 

Monday - Friday, Day shift, weekend rotation
