Position Summary: 

Provides professional nursing care to Adult Day Care participants and, when assigned, serves as backup coverage for the School Licensed Practical Nurse (LPN). Responsible for administering medications and treatments, performing comprehensive nursing assessments and ongoing reassessments, monitoring participant health, and ensuring compliance with all applicable agency policies and regulatory requirements. Provides clinical supervision and support to Licensed Practical Nurses (LPNs), Home Health Aides (HHAs), and Certified Nursing Assistants (CNAs), serves as a clinical resource for other Adult Day Care program locations, and assists leadership with quality improvement, regulatory compliance, staff education, and continuity of nursing services.


Duties and Responsibilities:

 

Adult Day Care:

 

  • Administer medications and perform treatments as noted in participant care plans or as directed by Director of Adult Day Services or Center Director.
  • Store medical equipment and medications appropriately.
  • Assist participants in all areas of toileting and personal care. Assess physically and document assessments.
  • Administer an oral hygiene program with those participants designated by the Director of Adult Day Services or Center Director.
  • Assist and supervise participants personal hygiene, including changing clothes, providing nail care, and providing hair care as necessary.
  • Be familiar with participant health status and report any unusual occurrences to the Director of Adult Day Services and Center Director.
  • Attend care plan meetings as scheduled.
  • Lead health maintenance programs, assess and assure ongoing hydration program.
  • Assist with daily activities.
  • Monitor participants special diets, as needed.
  • Assist participants with meals and snacks including, but not limited to, assisting with setup, coaching participants, feeding, etc. Routinely assess and document appetite, functioning level of self-feeding, etc.
  • Assist in loading and unloading vans as needed
  • Monitor room and walk with wanderers.
  • Understand the needs of aging adults and adults with disabilities and their caregivers.
  • Document participants progress. Forward care plan goals. Maintain current nursing/progress notes.
  • Practice environmental safety and sanitation.
  • Always follow Adult Day Care and Agency Health and Safety policies and protocols.
  • Provide clinical supervision and support to Licensed Practical Nurses (LPNs) at other Adult Day Care program locations, as assigned. May also provide nursing coverage at other agency centers when staffing needs arise or coverage is required. 


School (When Providing Coverage for the School LPN):

When assigned to provide coverage for the School Licensed Practical Nurse (LPN), the Registered Nurse (RN) shall perform the following responsibilities in accordance with all applicable school policies, accrediting standards, funding agency requirements, and agency policies and procedures:

  • Daily check of all children to determine health status and general welfare.
  • Contact parents and/or caregivers of children and clients if medical condition warrants concern and cannot remain at facility. 
  • Administer all medications and perform treatments as noted in student care plans and / or according to guidelines set forth by accrediting and funding agencies.
  • Deal with all medical emergencies.
    • Be familiar with the student health status and report any unusual occurrences to the Director.
  • Provide parents and caregivers individually and / or in group with information to include but not limited to nutrition, cleanliness, health, physical care.
  • Alert parents regarding renewal date for inoculations and if necessary, provide assistance to obtain inoculations needed by children.
  • Provide training in CPR, First Aide and universal precautions to staff.
  • Conduct monthly safety inspections.
  • Maintain a daily log of all nursing /medical issues and maintain current nursing/progress notes.
  • Store medical equipment and medications appropriately.
  • Monitor student special diets, as needed. 
    • Assist participants with meals and snacks including, but not limited to, assisting with setup, coaching participants, feeding. 
    • Routinely assess and document appetite, functioning level of self-feeding. 
  • Always follow the School Health policy and protocols.
  • Inform Director/Supervisor of any concerns or issues that involve the health and safety of the children immediately.
  • Attend in-services and other trainings as required


Other:

  • Provide clinical leadership during medical emergencies, disasters, and emergency preparedness activities across agency programs.
  • Other related duties as necessary to ensure the smooth running and efficiency of Easterseals South Florida as a whole.
  • Ensure the safety of all staff, volunteers, visitors and individuals receiving services at Easterseals.
    • Act in compliance with Medical and Non-Medical Emergency Procedures.
    • Assist individuals in evacuating building during emergency situations to include but not limited to fire drills and external disaster drills. Specific duties to be assigned by immediate supervisor.
    • Assume assigned responsibility for non-clinical duties during disaster and/or drills including but not limited to safety of files/office areas, closure of doors.
  • Work a flexible schedule as required (i.e. month end closing).
  • Participate in continuous agency wide quality and performance improvement and comply with all Quality Improvement requirements.
  • Adhere to the use of Universal Health & Safety Precautions as applicable

Clinical Leadership

  • Provide clinical guidance and support to LPNs, HHAs, and CNAs. 
  • Review nursing documentation for accuracy, completeness, and regulatory compliance. 
  • Ensure nursing practices comply with Florida Nurse Practice Act, AHCA, ASPEN, and agency policies. 
  • Assist with development, implementation, and evaluation of participant care plans.
  • Serve as the clinical liaison between the Adult Day Care programs, physicians, pharmacies, families, and other healthcare providers.

Oversight of Other Centers

  • Provide clinical oversight to nursing staff assigned to other Adult Day Care locations. 
  • Conduct periodic site visits or chart reviews, as assigned. 
  • Assist with orientation, competency validation, and ongoing training of nursing staff. 
  • Serve as a clinical resource for nursing staff across all Adult Day Care locations.
  • Provide consultation and support to Directors and program leadership regarding participant health concerns and nursing services.

Quality and Regulatory Compliance

  • Participate in quality improvement initiatives and performance improvement activities. 
  • Assist with regulatory surveys, audits, and corrective action plans. 
  • Monitor infection prevention and medication management practices. 
  • Assist with incident investigations and follow-up as appropriate.
  • Assist with development, review, and revision of nursing policies, procedures, and clinical protocols.
  • Assist the Director of Adult Day Care Services with nursing operations. 
  • Monitor medication records, physician orders, and nursing documentation for compliance. 
  • Coordinate communication with physicians, pharmacies, therapists, and other healthcare providers.
  • Provide education and clinical training to nursing staff and direct care staff. 
  • Assist with competency assessments and annual clinical skills validation.


PHYSICAL DEMANDS: Light and occasionally heavy work.

  • Light work involves exerting up to 25 pounds of force occasionally to lift, carry, push, pull, and otherwise move objects. Involves bending, squatting, reaching, and moving about most of the time.
  • Heavy work involves exerting up to 50 pounds of force occasionally to lift, carry, push, pull, and otherwise move objects. Involves bending, squatting, reaching, and moving about most of the time.


EQUIPMENT OR MACHINERY REQUIRED TO OPERATE includes, but is not limited to: 

  • Standard medical equipment used within their licensed scope of practice, standard equipment used in rehabilitation facilities, including wheelchairs, walkers, etc.; standard office equipment, including, computer, telephone, fax machine, copy machine, and calculator.

 

Educational & Experiential Requirements: 

 

  • Must possess an active Florida Registered Nurse license.
  • Must possess CPR/ First Aid Certification.
  • Minimum of 2–3 years of nursing experience (geriatric, adult day care, long-term care, rehabilitation, or community health preferred).
  • Previous supervisory or leadership experience preferred.
  • Experience with dementia care and chronic disease management preferred.
  • Knowledge of AHCA regulations, ASPEN Standards of Care, and Florida Adult Day Care licensing requirements preferred. 
  • Ability to act quickly and calmly during stressful and/or emergency situation. 
  • Ability to effectively communicate verbally with a diverse population. 
  • Ability to promote teamwork and build effective relationships.
  • Ability to take initiative and meet objectives.
  • Ability to physically assist participants in care such as pushing and manipulating wheelchairs, bathing assistance, dressing assistance, transferring assistance and during emergency situations
  • Ability to maintain confidentiality regarding personal information of participants and staff

Reporting to this position: 

Home Health Aides (HHAs), Certified Nursing Assistants (CNAs), and Licensed Practical Nurses (LPNs) assigned to Adult Day Care Services and the School, including those at other agency program locations, as applicable.
