DOCUMENTATION: Prepares written reports and clinical documentation according to agency, program and practice guidelines. Submits PCAS, SCEIS, and productivity on time per agency requirement. Submits program and social work reports as specified and by designated deadlines.
COMMUNITY OUTREACH: Participation in agency and community outreach activities to enhance efforts to reduce the impact of STD/HIV/AIDS/Viral Hepatitis.
CQI/PROFESSIONAL DEVELOPMENT: Completes required agency trainings and emergency preparedness exercises. Participates in quality improvement activities and social work meeting to improve effectiveness of social work services and program activities. Maintains active professional licensure. Completes competency evaluations and schedule audits as designated by DPH professional practice and program policy.
Leadership: The SWM assures accountability for social work practice within their chain of command. Provides basic SW orientation, ensures SW licensure verification annually, conducts regularly scheduled chart audits, and reports SW practice concerns to SW Director, convenes SW meetings, facilitates SW continuing education opportunities, conducts SW programmatic audits, and provides SW practice oversight. Serves as the DBHTL. Serves as backup during absence or vacancies. Integrates and coordinates social work activities with social work staff and those of other disciplines, Participates in the hiring and orientation process of new SW. Provides field instruction/preceptorship for SW students/interns when needed. Participates in leadership growth opportunities as they present. Completes supervisory duties consistently and on time.
OTHER DUTIES AS ASSIGNED: Will assist with social work referrals in other program and geographical areas when needed. If necessary, the social worker may be TB Screened/Fit Tested and be able to use appropriate respiratory protective equipment to assist with a large-scale TB outbreak investigation or to provide urgent, in person SW services to TB clients as requested. Participates as a part of an area Disaster Behavioral Health Team (DBHT) in an emergency or major disaster to expedite services to the public as outlined in the DPH/County plans. During such times, the incumbent should regard him/herself as on 24-hour call and subject to duty when such events occur or are anticipated to occur. During an emergency event, may serve as the DBHTL to provide additional assistance and leadership as directed. Will assist with other duties as assigned.
