Summary:
The Licensed Practical Nurse - Telehealth Coordinator supports facility patient care using telehealth technologies which increases the ability for patients to see community specialty providers while lessening the demand for transport by custody staff. This includes identifying and working with specialty providers to determine the types of encounters that can be conducted via telehealth and then scheduling the appointment.

 

Primary Duties and Responsibilities:

  • The Licensed Practical Nurse - Telehealth Coordinator works collaboratively with the designated patient scheduler to arrange all medical telehealth appointments for inclusion on daily call-out lists.
  • Prepares the clinical environment, which may include preparing medical and telecommunications equipment to be used for remote clinical examination and treatment of patients.
  • The Licensed Practical Nurse - Telehealth Coordinator assists patients with the completion of standardized patient forms and history, as may be required by the community specialty provider, prior to, during or following the telehealth encounter.
  • Observes and participates in all telehealth encounters and serve as a liaison to the community specialty provider in communicating current vital signs, patient education or other information requested by the community specialty provider.
  • Provides records as requested to community specialty Providers in advance of telehealth appointments and shares pertinent medical data/summary from electronic medical record with community specialty provider, as requested, during telehealth encounters.
  • The Licensed Practical Nurse - Telehealth Coordinator may obtain consent for treatments or medications but do not chart the contents of the encounter into the electronic medical record.
    The community specialty provider will provide a summary of the patient encounter for scanning/uploading into the electronic record, just as if they patient were seen in the Providers community office
  • Prepares monthly statistics of telehealth appointments and provides this data to the facility Health Services Administrator.
  • When no appointments are scheduled, may assist with floor nursing operations and pill calls.
  • Assess telehealth capabilities, systems and potential enhancements.
  • Coordinates intake-related laboratory duties, including specimen collection support, requisition completion, lab tracking, timely follow-up, and escalation of abnormal or critical results to the RN, provider, and Health Services leadership as applicable.
  • Coordinates intake-related diagnostic services and continuity of care, including EKGs, imaging, and other ordered diagnostic studies; ensuring results are obtained, routed for provider review, and documented in the health record.
  • Reviews intake records for pending laboratory or diagnostic orders and communicates outstanding results, referrals, or required follow-up to the appropriate clinical staff to support timely patient care.
  • Maintains accurate and complete intake medical records by verifying required forms, screenings, consents, medication history, allergies, vital signs, provider orders, and follow-up documentation are completed and entered, scanned, or filed in the electronic health record as required.
  • Supports medical records duties by tracking incomplete intake documentation, communicating deficiencies, assisting with record reconciliation, and maintaining confidentiality of protected health information in accordance with company policy and applicable regulations maintaining confidentiality of medical records.
  • Performs other duties as assigned.

Minimum Requirements:

  • Graduate of an accredited school of nursing required. Must have a current, unrestricted license to practice as a Licensed Practical Nurse (LPN) in the state the facility is located.
  • Maintains a current copy of license on file at the facility at all times.
  • Two (2) years of direct work experience Licensed Practical Nurse preferred.
  • General knowledge in the use of telemedicine equipment and process preferred.
  • Must possess competence in the performance of the following services: crisis intervention, medical emergencies, physical assessment, medical nursing care, and psychiatric nursing care.
  • Must have the ability to work without direct, immediate supervision in performing nursing diagnosis, and be able provide appropriate follow through treatment.
  • Ability to work with computers and the necessary software typically used by the department to accommodate telehealth encounters, as well as electronic medical records.
