What You’ll Do

Support Person-Centered Planning & Care

  • Establish objectives for the Person-Centered Individualized Support Plan (PCISP) goals 
  • Assist with the development and approval of Behavior Support Plans (BSPs)
  • Prepare high-risk plans in partnership with the Wellness Coordinator (RN)
  • Write individualized protocols and complete monthly and quarterly reviews

 

Ensure Quality, Safety & Compliance

  • Report and present information for Human Rights Committee meetings
  • Ensure utility bills, leases, and documentation are handled correctly
  • Review end-of-month documentation for accuracy
  • Attend appointments and administer medications as needed

 

Lead & Support DSP Teams

  • Provide guidance and support to Direct Support Professionals (DSPs)
  • Conduct performance evaluations
  • Manage attendance, approve timecards, and assign attendance points
  • Assist DSPs with organizing shift responsibilities

 

Provide Coverage & Support

  • Fill open shifts and provide shift coverage when needed
  • Provide on-call support based on service site needs

Requirements

  • High school Diploma / GED preferred. 
  • Bachelor's degree in a Human Service field or education preferred.
  • Experience in direct support service and/or working with intellectual disabilities preferred.
  • Meet background check requirements
  • Must provide:
    • Own transportation
    • Have a valid driver’s license
    • Vehicle registration 
    • Proof of insurance 
    • Acceptable driving record
