What You’ll Do
Support Person-Centered Planning & Care
- Establish objectives for the Person-Centered Individualized Support Plan (PCISP) goals
- Assist with the development and approval of Behavior Support Plans (BSPs)
- Prepare high-risk plans in partnership with the Wellness Coordinator (RN)
- Write individualized protocols and complete monthly and quarterly reviews
Ensure Quality, Safety & Compliance
- Report and present information for Human Rights Committee meetings
- Ensure utility bills, leases, and documentation are handled correctly
- Review end-of-month documentation for accuracy
- Attend appointments and administer medications as needed
Lead & Support DSP Teams
- Provide guidance and support to Direct Support Professionals (DSPs)
- Conduct performance evaluations
- Manage attendance, approve timecards, and assign attendance points
- Assist DSPs with organizing shift responsibilities
Provide Coverage & Support
- Fill open shifts and provide shift coverage when needed
- Provide on-call support based on service site needs

Requirements
- High school Diploma / GED preferred.
- Bachelor's degree in a Human Service field or education preferred.
- Experience in direct support service and/or working with intellectual disabilities preferred.
- Meet background check requirements
- Must provide:
- Own transportation
- Have a valid driver’s license
- Vehicle registration
- Proof of insurance
- Acceptable driving record
